Getting Started
A written referral from a primary care physician is necessary before a child may be evaluated. Doctors, therapists, school systems, and early childhood intervention programs often suggest such a referral. Our office will fax this form to your physician upon your request.
Once a family contacts our clinic, we send a packet of information that includes a questionnaire to be completed by the family or guardian. After we receive the completed questionnaire, we schedule an appointment for an in-depth evaluation, which results in a detailed written report.

Insurance: We accept and file insurance, including Medicaid, for eligible children. We will also contact your insurance company to determine coverage for services.
